By booking an appointment with Flourishing Futures, you agree to the terms below. This outlines what you can expect from us and your responsibilities as a client.
1. Our Services
Flourishing Futures provides:
- Occupational therapy assessments in your home, care home, local environment, school, workplace or online
- Tailored therapy programmes, reports, and recommendations
- Person-centred therapy that is safe and supportive, but does not replace medical advice from your GP or other healthcare professionals
2. Your Responsibilities
As a client, you agree to:
- Provide accurate health and personal information
- Attend appointments on time, or give at least 48 hours’ notice if cancelling or rearranging
- Follow guidance responsibly and ask questions if anything is unclear
3. Fees, Payment & Cancellations
- Fees are discussed and agreed at the time of booking.
- A free 15-minute introductory consultation may be offered to discuss needs and suitability.
- Self-funding clients: Payment is usually required in advance or at the time of the appointment.
- Care homes, agencies, or organisations: Invoicing can be arranged by prior agreement.
- At least 48 hours’ notice is requested for cancellations or rescheduling. Where less notice is given, or an appointment is missed, the full session fee may apply, except in exceptional circumstances.
- If Flourishing Futures cancels due to unforeseen circumstances (e.g., severe weather or emergency), we will aim to provide as much notice as possible and reschedule at a convenient time.

4. Confidentiality, Data & Policies
- Your personal and health information is kept confidential and stored securely.
- Information is shared only when required by law or to provide your occupational therapy.
