Single Issue Assessment – £330–£375
A focused 60–90 minute assessment for one clearly defined concern, with a practical written report
- Observation and functional assessment in the most relevant setting (home, school, or community)
- Review of current strategies and practical recommendations for immediate use
- Guidance on equipment, adaptations, or environmental changes where appropriate
- Personalised written report with findings and recommendations
- Suitable for children or adults with a specific functional concern (e.g., seating, feeding, falls, mobility, fatigue)

We provide specialist occupational therapy for older adults experiencing early or increasing frailty. Our approach is proactive, personalised, and focused on maintaining independence, preventing falls, and supporting meaningful daily life at home.

Specialist Frailty Review – £375
2-hour face-to-face home visit + travel, administration, and personalised frailty-focused report
Includes:
- Specialist frailty-focused functional assessment in the home
- Falls risk screening and confidence evaluation
- Detailed review of daily routines, energy, mobility, and environment
- Collaborative discussion with you and/or family to agree practical strategies
- Personalised written report with proactive recommendations
- Travel and report writing included
Ideal for: Older adults who want a detailed, specialist assessment of frailty and personalised strategies to maintain independence and prevent decline.
Independence Optimisation Programme – £650
Specialist Frailty Review + 3–4 follow-up therapy sessions (home or community-based)
Includes:
- Initial Specialist Frailty Review (as above)
- 3–4 tailored follow-up rehabilitation sessions to implement the recommendations
- Guidance and support for family members or carers
- Travel included
Ideal for: Older adults who want structured, goal-focused support to maintain strength, confidence, and independence following the review.
Proactive Support / Ongoing Monitoring – £85 per visit
Scheduled home or community visit (approx. 1 hour face-to-face) with optional follow-up call
Includes:
- 1 hour face-to-face visit
- Brief follow-up report/notes and recommendations
- Early intervention to address subtle changes and support independence
Ideal for: Individuals who want reassurance and structured follow-up to support ageing well at home. Additional sessions can be booked as needed.
Care Home & Provider Services
Flourish Platinum – £300 per month
A monthly retainer providing support, advice, and education for carers and care home staff:
- Telephone and email guidance during working hours
- Priority access to services when assessments or rehabilitation are required
- Up to 4 hours of included support per month (additional hours at £72.25/hr)
This service is designed to help your team with practical guidance, staff education, and problem-solving. By supporting your staff in this way, care homes can feel confident in providing safe, effective care and staff may identify opportunities for residents or families who would benefit from personalised occupational therapy services.
Education & Support for Care Homes
- Full day: £680
- Half day: £390
Education sessions are practical, hands-on, and tailored to your team’s needs. They can complement the monthly retainer or be booked independently.
Travel
All services are client-based. Travel is charged separately from clinical time:
Travel is charged at £60 per hour (to cover time and mileage).
Time is calculated using Google Maps estimated travel to and from our base to your location.
Services are provided within 90 minutes travel time from our company base. Distances beyond this will be quoted individually.
A full cost estimate will always be provided before your booking is confirmed
Payment
- A 50% deposit is payable at booking to secure your appointment.
- The remaining balance is requested electronically 1–2 days before the appointment.
